Online form - The easiest way to request a review is to complete and submit the online review form.

Download a form - If you prefer, you can download a copy of the review form to print and complete... and then fax, post or email to Health Link.

Speak to a consultant - To discuss your needs with a consultant, call Health Link on (03) 9670 5555 or Freecall 1800 808 026.

On receipt of your details, an experienced consultant will review your situation and prepare a personal policy recommendation for you. This will usually be emailed or posted to you within 48 hours.

Best of all, Health Link's review and recommendation service is free and without obligation to all University staff.
